Your first visit to Maria Jordan, MD, PA establishes a vital foundation for our relationship with you. During the first visit, we obtain important background information, like your medical history, and give you time to get to know your doctor. To understand what to expect for your first visit please read through this page. You will find useful information such as a map and directions to our office, practice hours, payment policies and more. As we update our patient registration forms online, these will be available for you to print and complete prior to your office visit.

We want to provide welcoming and calming surroundings, to make our patients' visits as pleasant as possible, even under the most stressful circumstances.

What to expect
Being well prepared for your appointment will help you and the doctor to make the most of your visit. Please bring a current medication and dosage list and/ or the bottles to every visit. This includes over the counter medications/ supplements. Please have the name and address of the pharmacy you wish to enter in your profile. Please bring the name and number of former doctor(s) from whom we may need to obtain records.
We strive to protect our patients' privacy and follow HIPPA guidelines. Please bring a valid photo identification card such as driver's license as this is required at registration. Although we do not file insurance, we also make a copy of your health insurance card to keep on file should it be necessary to call for prior authorization for medication approval.
